with Medicare Certification
The Manor at Hearthstone Communities is Medicare Certified and provides rehabilitative skilled therapy that promotes recovery from an illness or injury and
return to the home environment. The Manor at Hearthstone Communities can accommodate both
short and long-term rehabilitations.
 Rehabilitative nursing care is available to help sustain gains made with the
licensed therapist. Our goal is to promote the highest level of
independence in each individual.
A rehab area with strength training equipment is on site.
Scheduled visits are made by primary care physicians, a dentist, optometrist, podiatrist
and other medical professionals.
